Chronic Hepatitis C is the leading cause of chronic liver disease in advanced final stage of hepatocellular carcinoma (HCC) and of death related to liver disease. Evolves progressively in time 20-30 years. Evolutionary rates vary depending on factors virus, host and behavior. This study evaluated the impact of hepatitis C on the lives of patients treated at a referral service in Hepatology of the University Hospital Onofre Lopes - Liver Study Group - from May 1995 to December 2013. A retrospective evaluation was performed on 10,304 records, in order to build a cohort of patients with hepatitis C, in which all individuals had their diagnosis confirmed by gold standard molecular biological test. Data were obtained directly from patient charts and recorded in an Excel spreadsheet, previously built, following an elaborate encoding with the study variables, which constitute individual data and prognostic factors defined in the literature in the progression of chronic hepatitis C. The Research Ethics Committee approved the project. The results were statistically analyzed with the Chi-square test and Fisher's exact used to verify the association between variable for the multivariate analysis, we used the Binomial Logistic regression method. For both tests, it was assumed significance p < 0.05 and 95%. The results showed that the prevalence of chronic hepatitis C in NEF was 4.96 %. The prevalence of cirrhosis due to hepatitis C was 13.7%. The prevalence of diabetes in patients with Hepatitis C was 8.78 % and diabetes in cirrhotic patients with hepatitis C 38.0 %. The prevalence of HCC was 5.45%. The clinical follow-up discontinuation rates were 67.5 %. The mortality in confirmed cases without cirrhosis was 4.10% and 32.1% in cirrhotic patients. The factors associated with the development of cirrhosis were genotype 1 (p = 0.0015) and bilirubin > 1.3 mg % (p = 0.0017). Factors associated with mortality were age over 35 years, abandon treatment, diabetes, insulin use, AST> 60 IU, ALT> 60 IU, high total bilirubin, extended TAP, INR high, low albumin, treatment withdrawal, cirrhosis and hepatocarcinoma. The occurrence of diabetes mellitus increased mortality of patients with hepatitis C in 6 times. Variables associated with the diagnosis of cirrhosis by us were blood donor (odds ratio 0.24, p = 0.044) and professional athlete (odds ratio 0.18, p = 0.35). It is reasonable to consider a revaluation in screening models for CHC currently proposed. The condition of cirrhosis and diabetes modifies the clinical course of patients with chronical hepatitis C, making it a disease more mortality. However, being a blood donor or professional athlete is a protective factor that reduces the risk of cirrhosis, independent of alcohol consumption. Public policies to better efficient access, hosting and resolution are needed for this population.

The present study analyzes the conceptions and the nursing workers' practices about the relationship between vaccination and biosecurity in a public hospital of reference in communicable diseases in Natal/RN. It is treated, therefore, of a exploratory/descriptive study with qualitative and quantitative approach. They were constituted as collaborators of the research twenty-two nursing professionals, being five nurses, three auxiliary and fourteen technicians in nursing. The information were obtained through the interview technique with semi-structured route. In the quantitative approach, the information were analyzed by statistics and presented in form of tables and graphs to characterize the workers and the aspects related to the vaccination situation of the same ones; and in the qualitative approach, we used the method of content analysis. The analysis was accomplished starting from the categories empiric coming from the process of analysis of the field material, measure through inferences and interpretations based on the authors studied in the theoretical referential of the research. Starting from the analysis of the results, we verified that the nursing interviewees' workers establish in a clear way and it aims at, in your speeches, a direct relationship between vaccination and biosecurity besides attributing a meaning of great importance with relationship to the use of the vaccines in your professional lives in what refers to the control and decrease of the risks, above all the biological ones, to the which are exposed in the daily exercise of your functions. However, when analyzing those workers' vaccination situation, we verified that the vaccination covering still meets on this side of the expected for the vaccines of occupational interest, with prominence just for the vaccines against diphtheria and tetanus and to against hepatitis B, that presented coverings considered very good and above the national average. Considering that the institution, although has a service of health occupational active and offer some vaccines of occupational interest, it still presents a work of little mobilization in what refers to the consciousness and the workers' permanent education with relationship to the need and importance of the occupational vaccination, not only for your workers' protection, as well as measure in the infection control and, therefore, as safety for your patients/clients. We understood that that work type didn't still become politics guided by ministries of Health or of the Labor, however, it falls to the institutions that work for the interest of the workers' health to struggle for all and any action and mobilization that have as objective protects the workers of the risks in your work atmosphere

In this study, a BCR-ABL expressing human chronic myelogenous leukaemia cell line (K562) was used to investigate the antitumoral potential of a novel lectin (CvL) purified from the marine sponge Cliona varians. CvL inhibited the growth of K562 cells with an IC50 value of 70 g/ml, but was ineffective to normal human peripheral blood lymphocytes in the same range of concentrations tested (180 g/ml). Cell death occurred after 72 h of exposure to the lectin and with sign of apoptosis as analysed by DAPI staining. Investigation of the possible effectors of this process showed that cell death occurred in the presence of Bcl-2 and Bax expression, and involved a caspase-independent pathway. Confocal fluorescence microscopy indicated a major role for the lysosomal protease cathepsin B in mediating cell death. Accordingly, pre-incubation of K562 cells with the cathepsin inhibitor L-trans-epoxysuccinyl-L-leucylamido-(4-guanidino)butane (E-64) abolished the cytotoxic effect of CvL. Furthermore, we found upregulation of tumor necrosis factor receptor 1 (TNFR1) and down-modulation of p65 subunit of nuclear factor kappa B (NFB) expression in CvL-treated cells. These effects were accompanied by increased levels of p21 and downmodulation of pRb, suggesting that CvL is capable of cell cycle arrest. Collectively, these findings suggest that cathepsin B acts as death mediator in CvL-induced cytotoxicity possibly in a still uncharacterized connection with the membrane death receptor pathway

The Back School has been used to prevent and to treat back pain since 1969, however its effectiveness still is controversy in literature. The objective of this study was to evaluate the effectiveness of a program of "Back School" in patients with nonspecific chronic low back pain, directed by rheumatologists and orthopedic doctors to Physiotherapy School Clinic of Universidade Potiguar (UnP) Natal/RN- Brazil, in period of May/2002 to December/2003. Seventy patients, with age varying from 18 to 60 years, were randomized in two groups: Experimental group (group A) with 34 patients, which was subdivided in groups with 6 to 8 components. This group participated of a theoretician and practical program of Back School with 4 lessons, one day per week, with 60 minutes of duration; to the Control group (group B), with 36 patients, was explained that the group should stay four months in a waiting list . Both groups, had been carried through three evaluations, by a blind, to patient group, independent observer: initial evaluation, after four and sixteen weeks. The following variables were analyzed: pain intensity, measured by analog visual scale (AVS), functional disability, measured by disability questionnaire of Roland and Morris and the spinal mobility measured by Schöber s method. In statistical analysis it was used variance analysis ANOVA, the test of Newman-Keuls multiple comparations, and the Pearson correlation coefficient, with significance level p<0.05. Thirteen patients (18,6%) didn t complete the evaluations, (5 from experimental group and 8 from control group). At last, 57 patients were studied, (29 from Experimental group and 28 from Control group), it was observed a significant statistical improve just for Experimental group in variables pain intensity (p=0,0001), functional disability

Chronic lymphocytic leukemia (B-CLL) is a clonal proliferation of mature B lymphocytes characterized by indolent clinical course. Biologically this clonallity is characterized by low expression of surface immunoglobulin (sIg) with restriction to a single immunoglobulin light chain associated with high expression of CD5 antigen and positivity to B cell antigens lymphocytes such as CD19, CD20 and CD23 and negativity to FMC7. The immunological profile and morphological analysis of lymphoid cells are the main means for the differential diagnosis of B-CLL from other chronic lymphoproliferative diseases. The aim of this study was to evaluate the expression pattern of a variety of membrane antigens in leukemic cells originating from patients with B-CLL. In this study, peripheral blood samples from 80 patients with B-CLL were analyzed by multiparametric flow cytometry in addition to routine hematologic exams, using a panel of monoclonal antibodies (MoAb): CD45/CD14, CD3/CD19/CD45, CD4/CD8 / CD3, CD20/CD5/CD3, CD3/CD16-56/CD45, CD2/CD7, FMC7/CD23, CD103/CD22/CD20, HLADR/CD38, CD10/CD19, CD1a, CD11b and also IgM/gD, kappa and lambda immunoglobulin light chains for the detection of surface immunoglobulin and clonal restriction for immunoglobulin light chain. The Hematological data were obtained from the hematological analyzer and cytomorphological analysis in blood film stained by Leishmann. The study samples consisted of 45 men and 35 women, ages ranging from 55 to 84 years (mean 65 years). Complete white blood count showed count ranging from 10.0 to 42.0 x 109/l. (mean 50.0 x 109/l) and lymphocytes count greater than 5.0 x 109/l in all cases. The neoplastic cells displayed B-CLL phenotype (CD5+/CD19+/CD20+/HLADR+/CD23+) in the vast majority of the cases, associated to failed to stain for T cell markers (CD1a, CD2, CD4, CD3, CD7, CD8), CD103, CD14 and FMC7. Leukemic cells of most patients also expressed low intensity of IgM and IgD with restricted kappa light chain, in most cases (59,7%). This observation highlights the importance of immunophenotyping for correct diagnosis of chronic lymphoproliferative syndromes and the panel of MoAb used was sufficient for diagnostic confirmation of B-CLL

A Anfotericina B (AmB) é amplamente utilizada no tratamento de infecções fúngicas sistêmicas. No entanto, a sua utilização é limitada devido a sua elevada toxicidade aguda e crônica. Os superagregados de AmB (H-AmB) obtido pelo processo de aquecimento controlado apresentam um potencial reduzido de toxicidade quando comparados à Anfotericina B micelar não aquecida (M-AmB). Diante do exposto, o objetivo deste estudo foi avaliar as características físico-químicas do processo de formação dos superagregados após a liofilização do H-AmB, por meio de técnicas tais como a calorimetria de exploratória diferencial (DSC), termogravimetria (TG) e análise térmica diferencial (DTG), ressonância magnética nuclear (RMN), espalhamento dinâmico de luz (DLS) e difração de raios X (DRX). Os dados de DLS indicaram um tamanho de aproximadamente 260 nm deste novo sistema, sendo este tamanho compatível e viável para administração intravenosa. A análise de DRX demonstrou a formação de um novo estado cristalino do sistema micelar, podendo este estar relacionado a presença de grande proporção de desoxicolato de sódio (NaDC) na formulação. O aquecimento do NaDC resulta na formação de uma estrutura helicoidal intermolecular, promovendo desta forma o aumento da agregação micelar e consequentemente aumento de seu tamanho. Os estudos das análises térmicas demonstraram que o processo de aquecimento não influência no comportamento das amostras. Os dados de RMN da H-AmB demonstraram a presença de ácido desoxicólico além do NaDC. O ácido desoxicólico é formado após o processo de aquecimento e sugere-se que o equilíbrio entre ambas as moléculas seja responsável pela redução da toxicidade da AmB. Os resultados aqui apresentados sugerem que o processo de aquecimento controlado altera a organização estrutural das micelas, resultando na diminuição da toxicidade, na melhoria da estabilidade térmica e na manutenção da atividade.
